Medical House Calls
Nurse Practitioners, Registered Dietitians and Therapies provide an old-fashion house call with a new twist. Services are mutually determined between your physician and Medical House Call providers.
- Facilitate earlier hospital discharge.
- Prevent unnecessary ambulance trips, ER visits or urgent care visits.
- Provide access to care when transportation, mobility, or other factors make it difficult or impractical to be seen at the clinic.

Nurse Practitioners
- Provide a "clinic" type visit in the home that may include:
- Chronic disease management
- Medication review, teaching, and prescribing
- Assessment, lab work, order x-rays as needed
Physical Therapists (PT) & Occupational Therapists (OT)
- Provide skilled evaluation and therapy procedures including:
- Exercise programs
- Transfers and ambulation
- Activities of daily living
- Use of equipment
- Modalities for pain relief
- Energy conservation techniques
Registered Dietitian
- Provide comprehensive nutrition assessment, dietary education and management of diabetes, cardiac disease, renal disease, would healing
- Enteral/Parenteral nutrition
- Weight management
- Medical nutrition therapy for Dysphagia

Telehealth
Daily home Telehealth monitoring assists in:
- Identifying patient's needs
- Detecting subtle changes in condition and early intervention
