Coordinating Services may include:

  • Needs assessment
  • Development of Care Plan
  • Coordinate assistance from paid service providers
  • Monthly contact with care teams
  • Quarterly in home visits
  • Monthly report to families

Monitoring Servcies may include:

  • Safety assessment of living arrangements
  • Assessment of alternative living arrangements
  • Assistance in completing applications for community resources
  • Advocacy at Medical and Financial appointments
