Mayo Clinic Health System – Home Health & Hospice offers a wide health care continuum of services that allow individuals to be in their chosen home or facility environment, while maintaining their independence and dignity. We employ a specially trained staff to provide services in the comfort of your home. We are a State Licensed, Medicare and Medicaid certified agency. We provide services for patients with payment coverage from government (Medicare and Medicaid), 3rd parties (insurance, managed care, workers’ compensation etc.), or directly by patients and their families.
Home care can be brief, as in the case of post-operative (surgery) care or long-term for those with a chronic illness and disability. Mayo Clinic Health System – Home Health & Hospice allows people to remain in their homes for as long possible.
Mayo Clinic Health System – Home Health & Hospice began by providing Intermittent service in 1984. Then we started our Hospice program 1988 and added our Supportive Care program in 1989. In 1994, Hospice of the Menomonie Area merged with us. The Agency provides home care to the following twelve counties: Barron, Buffalo, Chippewa, Clark, Dunn, Eau Claire, Jackson, Pepin, Rusk, Taylor, Trempealeau and St. Croix, as well as the communities of Elmwood and Glenwood City.

The Agency is governed by a Board of Directors appointed by its affiliate hospital - Luther Hospital. The Board designates an Executive Director to oversee the functioning of the Agency, its programs and staff.
Referrals for care are accepted from an array of sources including individuals and families, hospitals, nursing homes, physicians, clergy, and community and government agencies. The service is usually reimbursed by Medicare, Medicaid, private pay, or private insurance companies. Mayo Clinic Health System – Home Health & Hospice
